The wellness assessment tools give individuals a clear picture of their current state of health and fitness. The Lifestyle Questionnaire (LSQ) and the Functional Age Assessment (FAA) have been validated by thousands of our corporate clients. These automated assessment tools protect individual confidentiality while at the same time providing summary reports to the contractual client.

The Lifestyle Questionnaire.
The LSQ asks 35 questions which address health and lifestyle issues. After the individual completes the form, the LSQ software generates a report including a summary of the results and a profile of eight risk areas—hypertension, body fat, smoking, daily activity, cholesterol, alcohol, back pain, and fatigue. Ideally, the fitness expert sits down with the individual to discuss the results. The LSQ assesses individual’s current fitness and health situation and their readiness to change risky behaviors. We will use this data to develop fitness programs tailored to individuals. The LSQ provides personalized recommendations for reducing health risks.

The Functional Age Assessment.
Functional age, more than chronological age, indicates a person’s overall fitness and health. A personal functional age is determined through the following: Resting heart rate, resting blood pressure, exercise heart rate, percent body fat, flexibility, grip strength, push strength and a
bdominal strength.

The FAA produces a report, which shows how the person compares with others of the same age. The report includes the percentile ranking, a graphical representation of the overall functional age, and the age score for each of the eight tests. The report also identifies areas for improvement. The FAA is a powerful tool for helping individuals assess their overall fitness and measure their progress. The LSQ and FAA software generates summary reports by gender, which profiles the overall health of the organization’s personnel/clientele.

The LSQ and FAA are tools for corporations and 3rd party health insurance administrators concerned with the health and fitness of their employees and clients.
